In accordance with Senate Bill 12, schools are now required to obtain written parental consent before providing any health-related services to students, except in life-threatening emergencies.
We have attached the 2025–2026 Health Services Consent Form for you to review and complete. This form allows you to opt in or opt out of specific health services for your child.
It is very important that you complete and submit this form as soon as possible. If you choose to opt out of a service, the school nurse will not be able to provide that service for your child, even if needed during the school day.

De acuerdo con la Ley del Senado 12 (Senate Bill 12), las escuelas ahora están obligadas a obtener consentimiento por escrito de los padres/tutores antes de brindar cualquier servicio relacionado con la salud a los estudiantes, excepto en emergencias que pongan en peligro la vida.
Adjunto encontrará el Formulario de Consentimiento de Servicios de Salud 2025–2026 para que lo revise y complete. Este formulario le permite autorizar (opt-in) o rechazar (opt-out) servicios específicos de salud para su hijo(a).
Es muy importante que complete y envíe este formulario lo antes posible. Si decide rechazar (opt-out) un servicio, la enfermera escolar no podrá brindar ese servicio a su hijo(a), incluso si lo necesita durante el día escolar.
